Healthy Start, Healthy Families


Healthy Start and Healthy Families

Healthy Start may provide free medical benefits to children up to ages 0-19 and pregnant women.

Healthy Families may provide healthcare for the entire family.

 Both Healthy Start and Healthy Families cover a variety of medical services, including:

  • Hospital stays
  • Doctor visits
  • Dental care
  • Vision care
  • Mental health treatment and counseling visits
  • Drug and alcohol treatment
  • Prescriptions
  • Prenatal care

Eligibility

If you applied for Healthy Start or Healthy Families in the past and were found ineligible, you may now be covered (see Income Guidelines below). You may be eligible even if you own a home or car and have a job or medical insurance.

How to Apply

Applying for benefits is easy. No face-to-face interview is required. Once found eligible, the Summit County Department of Job and Family Services will re-determine your eligibility for both the Healthy Start and Healthy Families programs every 12 months. It is not necessary to reapply, although you will need to re-verify your income annually and report any household and income changes as they occur. The following information is necessary to process your application:

  • Verification of all income for the last month
  • Written proof of pregnancy (from a licensed medical professional)
  • Any medical bills for the past 90 days
  • Proof of any other medical insurance
  • Photo identification (driver’s license, state ID, school ID)*
  • Proof of citizenship (birth certificate, hospital record, U.S. passport)*

*Please note: Photo identification and proof of citizenship must be an original document. Photocopies are not acceptable.
